Position Summary The incumbent fulfills the primary responsibilities involved coordinating the multidisciplinary approach in the evaluation, treatment and facilitation of heart failure patients, and the follow-up care of those individuals. The position exists to coordinate and serve as a member of the cardiac team and acts as the coordinator who collaborates with the cardiologists, surgeons, and heart failure/transplant team members in the care of cardiac patients. The incumbent serves as a leader and consultant for internal and external education of patients, caregivers, family, staff, emergency responders and general public. The role coordinates overall care of patients by incorporating current heart failure standards of care, is one of collaboration and coordination and is both collegial and consultative to nursing leadership, primary nurses and other members of the health care team. This position will work closely with our Physicians, assisting with patient flow, follow up, and communication. Cardiac experience is a plus.
