Position Summary The incumbent fulfills the primary responsibilities involved coordinating the multidisciplinary approach in the evaluation, treatment and facilitation of heart failure patients, and the follow-up care of those individuals. The position exists to coordinate and serve as a member of the cardiac team and acts as the coordinator who collaborates with the cardiologists, surgeons, and heart failure/transplant team members in the care of cardiac patients. The incumbent serves as a leader and consultant for internal and external education of patients, caregivers, family, staff, emergency responders and general public. The role coordinates overall care of patients by incorporating current heart failure standards of care, is one of collaboration and coordination and is both collegial and consultative to nursing leadership, primary nurses and other members of the health care team. This position will work closely with our Physicians, assisting with patient flow, follow up, and communication. Cardiac experience is a plus. The desire to balance changing priorities and stay organized is essential. #LI- LVIUH

Qualifications for the Coordinator- Heart Failure Role

  • Requires a current Nursing license in the State of Indiana. BSN preferred.
  • Requires that the RN has graduated from a nursing program accredited through the ACEN or CCNE. Basic Life Support certification required. Other advanced life support certifications may be required per unit/department specialty according to patient care policies including ACLS.
  • Requires experience that provides evidence of clinical competency and leadership skills.
  • Requires clinical practice experience related to the program, strong organizational, writing, interpersonal and verbal communications skills.
  • Requires critical thinking skills, the ability to work independently as well as track and perform multiple tasks.
  • Requires the ability to manage projects from inception to completion.
